This is a guest-post in the form of a written interview with Dr Amal - a female cupping therapist based in Leicester, England

Please give us a short introduction about yourself, and in particular how you ended up being a hijama therapist?

My name is Amal I am a Hijama therapist and I am currently practicing Alhijama for women in Leicester. I am originally from Egypt I have a Masters Degree in Oral Surgery. Many years ago, one of my closest friends was treated for back pain with Alhijama, and the pain was completely relieved. This incident made me interested to read about Hijama to get more information about it. Then I was invited to attend a course on Hijama. This course was very beneficial. I learnt the relation between Hijama and Islam and how great is our Prophet Muhammed (peace and blessings of Allaah be upon him) who advised us to perform Hijama many centuries ago. I have learned the scientific method to do Hijama, the types of hijama (dry, dry with massage and wet hijama) and the specific points (areas) which correspond to each disease or condition.

What particular illnesses/ conditions (if any), do you specialize / had the most experience in?

I have been practicing Alhijama (cupping) for more than seven years. I have had the opportunity to practice Hijama in Egypt for several cases; I have practiced Hijama in Netherlands and the UK too. The percentage of curing for these cases have exceeded 90% depending on the cases itself and for many cases, the percentage of curing was 100%. The medical background I have helped me to diagnose the disease, decide the treatment plan for it and to decide which type of cupping is suitable to treat the disease or the condition. In addition, my medical background helps me to deal professionally with any unexpected incident that could happen during the treatment.

There is no particular illness I am specialized in, as Hijama is a cure for a wide range of disease and conditions. I have treated several cases and conditions, for instance diabetes, high blood pressure, rheumatoid, arthritis, back, shoulder and knee pain, headache, migraines, anaemia, sciatic nerve pain, involuntary urination, lack of ovulation for women, menstrual problems, endometriosis, and many other diseases, pain and other women health problems.

Also what would you say about using cupping as a general preventative therapy and how often would you recommend this (once a month? once/twice a year?)

I agree with this opinion. Firstly from a religious point of view and according to a hadeeth narrated by Jaabir (may Allaah be pleased with him), the Prophet (peace and blessings of Allaah be upon him) said:

“If there is any good in your medical treatments, it is in the knife of the cupper, drinking honey, or cauterization with fire, as appropriate to the cause of the illness, but I would not like to be cauterized.” (Reported by al-Bukhaari, 10/139)

On the other hand and from the medical and scientific points of view, our health and well-being, especially the immune system are reliant on the movement of blood and body fluids (hormones, lymphatic fluid, etc.). Hijama (cupping) is believed to stimulate flow of blood and lymph to the affected area. Its uses include relieving pain in the muscles, especially back pain from stiffness or injury; It causes tissues to release toxins, and stimulates the immune system and enhances the body’s defence. It influences the composition of blood as it increases red and white blood cells leading to the purification of blood. It is also beneficial in beauty therapy as it promotes the production of hormones and stimulates hair growth.

For preventive measures, Hijama could be done twice per year or once per month depending on the preference of the person himself, but some diseases, conditions and health problems need Hijama to be repeated once per month to achieve prominent improvement and curing.

Cupping (Hijama) In Its Time Is A Cure For Every Disease


Abu Hurairah (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "Whoever performs cupping (hijama) on the 17th, 19th or 21st day (of the Islamic, lunar month) then it is a cure for every disease." [Saheeh Sunan abi Dawud (3861)].

The Best Days For Cupping (Hijama)


Ibn Umar (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "Cupping (hijama) on an empty stomach* is best. It increases the intellect and improves the memory. It improves the memory of the one memorising. So whoever is going to be cupped then on a Thursday in the name of Allaah. Keep away from being cupped on a Friday, Saturday and Sunday. Be cupped on a Monday or Tuesday. Do not be cupped on a Wednesday because it is the day that Ayoub was befallen with the trial. You will not find leprosy except (by being cupped) on Wednesday or Wednesday night." [Saheeh Sunan ibn Maajah (3488)].

* 'alaa ar-reeq in Arabic means to fast until after being treated with cupping. Once the treatment of cupping has been completed, one may eat and drink.

As for the Islamic day and night, the night enters before the day. So at sunset on Tuesday, Wednesday night comes in. Cupping (hijama) is best performed during the daytime between the athaan of fajr and the athaan of maghrib because yawm in arabic means daytime.

Cupping (Hijama) Is The Best Of Remedies

Anas ibn Maalik (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "Indeed the best of remedies you have is cupping (hijama)…" [Saheeh al-Bukhaaree (5371)].

Abu Hurairah (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "If there was something excellent to be used as a remedy then it is cupping (hijama)." [Saheeh Sunan abi Dawud (3857), Saheeh Sunan ibn Maajah (3476)].

The Sunnah days for cupping (hijama) every month are when the 17th or 19th or 21st of the lunar month coincide with a Monday, Tuesday or Thursday. These are the best and most beneficial days for cupping (hijama). If one is not able to be cupped on 17th, 19th or 21st (coinciding with Monday, Tuesday or Thursday) then any Monday, Tuesday or Thursday of the month.

Dry and massage cupping may be performed at any time.

Please give us a short introduction about yourself, and in particular how you ended up being a hijama therapist?

Assalamualaikum, my name is Amin Shah and I provide the Hijama service right out of my apartment on Commonwealth Ave., in Boston, Massachusetts, USA. While I handle Hijama on the men, my wife Nusrat performs Hijama for the ladies.

I was born and raised in Malaysia, of Indian-Muslim origin. My grandparents migrated to Malaysia as Muslim traders in early 1900s. Though I come from a family of physicians, I am not one myself. However, even as a child, I have always been interested in natural healing remedies as opposed to drug based medications.

I'm primarily in the real estate business, helping Muslims buy their home with no interest using seller-financing. As a sideline activity, I perform Hijama and provide other proven natural health remedies.

I started providing Hijama services because I couldn't find any reliable Hijama service here in the New England area. For those who are not aware, New England comprises of six states in the US, namely Massachusetts, Connecticut, Rhode Island, New Hampshire, Maine and Vermont. There must be about 1 million Muslims living in this region.

I had Hijama performed on me several years ago by the brother from Buffalo, NY when he was visiting Boston. I immediately felt much better after the Hijama service. Then after several months, I had the desire to have it done again but when I called the person, his phone was disconnected and I could not get a hold of him. I tried asking people and doing research on the Internet to see if anyone else does it, but could not find anyone. That was weird to me.

How could it be that with so much health benefits coming from Hijama, and realizing that it’s a sunnah of the Rasullullah (saw), yet there was no one around to provide it? It was an unacceptable condition for me. So I decided that if there is no one who can provide this important service in a reliable manner, then I need to take it upon myself to provide it.

So I researched more and found a highly experienced Hijama practioner from my home country, Malaysia and learnt it from him. Not only did he teach me the procedure of Hijama, I also learnt the clean, hygienic way of doing Hijama. For instance, instead of using blades to make the incisions, we use lancets that are disposable after each use. All cups are also sterilized to ensure they are clean for the next patient. We also use gloves, and disinfection lotion to prevent infections to ensure a clean, safe procedure of Hijama.

What particular illnesses/ conditions (if any), do you specialize / had the most experience in?

When a patients comes in for the first time, we always do the 6 cups Hijama on his or her back on both sides of the spine. Two cups are placed on the back top shoulders which help cleanse out the toxic blood from the head, neck and shoulders. Two cups on the middle back that cleanse the toxic blood from the liver and kidney meridians. And two cups on the lower back that cleanse the toxic blood from the hips and reproductive areas.

We find this initial 6 cups on the back procedure provides an overall cleansing that's very beneficial to the patients. Many patients of mine have reported much relief of headaches, fatigue, back pain and even knee pain with this procedure.

After this initial 6 cups on the back Hijama, when the patient returns after 4 or 6 months later, we can do a more specialized location, like the head, or knee, etc.

Also what would you say about using cupping as a general preventative therapy and how often would you recommend this (once a month? once/twice a year?)

I recommend any person over 35 years old to do Hijama every 4 months. At the very least, they should do it twice a year. I tell my patients that formation of toxic blood is a natural occurrence of the body. Toxic blood is from the death of the red blood cells, and the death of white blood cells, and other waste materials like uric acid, etc. This accumulation is a natural process of the body. The key is to get rid of this toxic blood accumulation. And Hijama is the process to get rid of the toxic blood. It cannot be got rid of by sweating, urine or faeces or even acupunture. Only Hijama is effective in getting rid of the toxic blood. Thus its important to have Hijama performed on a regular and consistent manner every 4-6 months.

Is there anything else you would like to add?


I feel its important that all Muslims take it upon themselves to have Hijama performed on them regularly. It takes an hour or two per session and by doing it twice a year, a whole host of diseases that are starting to happen in our bodies can be eliminated. It’s a simple procedure, low cost, takes a little time and yet the health benefits are numerous.

I heard a hadith of the Rasullullah (saw) who have been reported to have said, "If one of you dies, and he did not do Hijama, then he himself has caused his death." (something like this more or less )

Hijama is an important sunnah of the Rasullullah (saw) that we must revive and bring it back. I like to see many other Hijama services offered in all major cities across the US where Muslim public can easily go and avail themselves of this great health procedure.

Another hadith of Rasullullahu (saw), "On the night of Isra and Miraj, every angel that I met told me, "Tell your ummat to do Hijama".

Please give us a short introduction about yourself, and in particular how you ended up being a hijama therapist?

I am a practising dentist. I have special interest in natural remedies. I was influenced by a friend who had cupping done in Madina Munawwarah. I researched about hijama and was able to complete the course here in the UK.

What particular illnesses/ conditions (if any), do you specialise / had the most experience in?

I am confident in treating conditions like hypertension, backaches, migraine.

And do you find that in your experience, the conditions/illnesses that you have come across respond well to cupping (hijama) therapy? And if so, what are they?

The above conditions respond well to a series of treatments (a one off cupping session does not give you the desired result). Patients report a significant feeling of wellbeing.

Also what would you say about using cupping as a general preventative therapy and how often would you recommend this (once a month? once/twice a year?)

Cupping or hijama as a sunnah is recommended in the spring and the autumn.

As a hijama therapist, how often do you get hijama done yourself, and what for (if any specific condition) and who do you go to to get it done (since it is not an easy therapy to self-administer)?

Hijama Treatment of an Acute Ankle Injury - a case study By Dr Rizwhan Suleman (Mchiro).

The following is a hijama case study kindly contributed by Dr Rizwhan Suleman from Cardiff, Wales (see previous post dated 3 May, 2009: A comparison of Hijama (Cupping Therapy) with Chiropractic).



Hijama Clinic Case Study 1:
Treatment of an Acute Ankle Injury

Case History
A 24 year old Caucasian male country yard worker presents with an acute left ankle sprain injury. Onset was 5 days prior during an epileptic seizure. The exact mode of injury was unknown; the patient noted diffuse ankle swelling gradually over a period of half an hour, with no noticeable bruising or discolouration. Since the injury the patient has been wearing an ankle support and has been using ice but has not reduced his daily activities which involve 2-3 hours of walking. The patient noticed the swelling was progressing distally to the toes and proximally up the shin. He also described an insect bite between the toes, which occurred after the injury, as being infected.

Previous Medical History
The patient suffered encephalitis at the age of 17 and made a full physical recovery following a 2 month state of comatose. Subsequently the patient suffers with unprovoked seizures that have been gradually decreasing in frequency over the years, from 8 per day to the current, 2 per week.

Previous Treatment
The patient saw his GP who prescribed non-steroidal anti inflammatory medication for the injury and anti-histamine and antibiotics for the infected insect bite. X-rays were taken and reported no abnormalities.

Medication
Antiepileptic medication, NSAIDs, anti-histamine and antibiotics.

Physical examination
Blood Pressure, heart rate, respiratory rate and temperature were all within normal range. The patient was 5ft 10, slim built and walked with an antalgic gait. There was diffuse swelling of the left foot, ankle and distal shin when compared to the right. There was a pea sized septic skin lesion between the dorsal surface of the 1st and 2nd distal metatarsal heads (see figure 2). There were no obvious structural deformities. Range of motion was markedly reduced in the ankle joint and produced pain on both active and passive motion. There were heightened pain levels on inversion of the foot. Muscle testing gave sufficient resistance in all planes. Vibration testing was negative on all bony structures. Soft tissue palpation elicited focal tenderness over the lateral ankle ligaments.


Diagnosis
Acute grade 2 lateral ankle sprain

Treatment
In assessing the risk of a treatment initiated seizure, the patient was laid supine on the floor and a dry vacuum cup was applied to the hand. In the absence of any prodromal symptoms, a further cup was applied to the asymptomatic foot. With the patient reporting no adverse symptoms, wet cupping (hijama) treatment was initiated on the symptomatic foot. Due to the degree of swelling several medium to small size cups were applied to the foot and ankle in consecutive sets of 2-3. Positioning of the cups was aimed at reducing swelling in the toes, ankle and shin but was also influenced by the irregular surfaces of the foot and ankle making cup application difficult (see figure 1a & 1b). A single cup was applied to the site of the infected bite which aspirated a quantity of green/yellow fluid. General RICE treatment advice was given and the ankle dressed.


Post Treatment follow-up
Immediately after treatment there was a noticeable reduction in swelling (see figure 3). The patient reported feeling less pain and had a slight increase in ROM. On 24 hr follow-up there was a marked reduction in swelling, pain and ROM (see figure 4). On 1 week follow up the reduction in swelling was maintained and the patient was able to walk on the ankle with only a small amount of pain.


Note
Wet cupping (Hijama) in this case was an effective treatment in aiding the recovery of an acutely sprained ankle. While reviewing the different treatment options for this type of injury, it becomes apparent that there are few options that can claim to give such an accelerated progress in its management. In the context of sporting injuries where recovery time is of upmost importance this treatment may become a preferred option. Further case studies of a similar nature will help gain more insight into the benefits of wet cupping (Hijama) in such contexts.
