Inflammation, your best friend and worst enemy

The following is an interview with Dr Rizwhan Suleman (Mchiro) of Hijama Centres UK transcribed from his video, A Short Lesson in Hijama.




Inflammation, your best friend and worst enemy


- For those who have never heard of hijama, please give a background of hijama therapy

Hijama is known as cupping in English. It is one of the oldest medicines ever to be known by mankind. It was practiced by the ancient Egyptians, revered  by Galen “The Father of medicine”, and is used in Traditional Chinese Medicine. It is performed throughout Africa, well recognised in Arabia, but most importantly for Muslims, it was prescribed upon the Prophet Muhammad ( (صلى الله عليه و سلم))

"On the night of Israa I did not pass an angel except it said to me, “Oh Muhammad! Order your Ummah with Hijama” - Tirmidhee

"If there is any good in your medical treatments it is in Hijama” - Bukhari

He  (صلى الله عليه و سلم) used it for hip pain, when he sprained his ankle, for detoxification of the blood, and after ingesting poison. He prescribed it for migraine headache and as a maintenance of health

- Narrated Abu Kabshah al-Ansari: The Apostle of Allah (صلى الله عليه و سلم) used to have himself cupped on the top of his head and between his shoulders, and that he used to say: 
If anyone pours out any of his blood, he will not suffer if he applied no medical treatment for anything. (Book 28, Number 3850) Sunan Abu Dawud.

What does it involve and how is it done?


Suction is the basic principle. A vacuum is created in valved suction cups using a handheld suction pump. The cup is placed over the skin & air is sucked out. After removing the cup small cuts are made onto the skin. The cuts are very light and superficial and comparable to a finger prick blood test. Although subjective pain is very minimal requiring no local or general anaesthetic. Once done the cups are re-applied with suction. This slowly encourages the small cuts to bleed. The cups are left on for 5-10 minutes and small quantities of blood begin to collect into the cups. After a given time period waste blood is safely discarded and bleeding stops almost immediately

Marks left after treatment normally take less than 2 weeks to completely disappear.

But how can bleeding possibly be beneficial?


First let’s look at our most common health issues: INFLAMMATION: Your Best Friend & Worst Enemy. In any injury our bodies send more blood to the site. This is known as vasodilatation and is part of inflammation. The body’s defences and emergency services get mobilised to the site.This aids in repair, healing and protection of the area. 

So what’s the problem? Well, all our body tissues and cells are in a closed space under constant positive pressure. For there to be more blood in an area than there already is, more space is needed. But our bodies are enclosed in skin, tissues and fascia so more fluid equals more pressure. Increased pressure causes increased tissue stress, stretching and this becomes a swelling potentially causing more tissue damage.

Increased tissue pressure, swelling and stagnation equals pain and dysfunction of tissues & organs for example:
- carpal tunnel syndrome
- inflamed appendix
- inflammation of the liver
- inflamed disc in the spine causing nerve pressure
 - swollen knees and ankles


If the problem does not resolve swiftly a perpetual cycle of damage and tissue pressure can begin. As the pressure in the affected tissue rises its blood supply becomes gradually restricted allowing accumulation or hindered eradication of waste, acidity (CO2) and dead cells. This leads to poor health of the tissue, stretching, irritation and damage of pain receptors and eventually illness, pain and disease.

Controlling inflammation is medicines greatest challenge, as so many conditions are caused from it, for example any condition ending in ‘itis’ implies inflammation::
  • arthritis
  • gastritis
  • meningitis
  • asthma
  • colitis
  • bronchitis
  • tendonitis
  • celuclitis
  • peritonitis
  • vasculitis
  • neuritis
  • pericarditis
and many more


This is why anti-inflammatory medication is so commonly used.

So what does hijama have to do with it?


The negative pressure of hijama can draw blood from deep within tissues and organs using the body’s own transport mechanisms to the surface of the skin. The small cuts allow the release of any offensive stagnant blood/fluid venting the pressure build-up. Only stagnant, pressurised blood is released. The removal of inflamed, acidic, toxic blood, lymph and debris allows space for fresh nutrient rich blood to penetrate the once congested tissue. Blood saturated with waste proteins, acidity, inflammatory modulators and dead cells is removed.

Removing pressure and swelling from the tissues removes physical and chemical stimulation of nociceptors (pain receptors) and gives instant relief, fresh blood, oxygen and nutrients which allows the body to repair, regain health and for tissues to regain function.


It’s also believed to have many positive effects to the cardiovascular system, general immunity, organ function and neurological activation. Research shows it is effective for: back pain, migraine, bronchial asthma, rheumatoid arthritis, knee pain, blood purification and general wellbeing.More research is being conducted, clinical cases show remarkable results for a wide range of conditions.

But is it safe?


Under the guidance of a qualified health professional it is very safe. Patients are screened and assessed before treatment. All equipment is single use and disposable. Practitioners will advise on individual suitability as treatment is not advised for all cases.

Please kindly note that these dates are based on the new, lunar month according to the sighting of the new moon by the naked eye, as cupping (hijama) is a form of worship. These dates may sometimes vary from the calculated, lunar dates.

Cupping (Hijama) In Its Time Is A Cure For Every Disease


Abu Hurairah (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "Whoever performs cupping (hijama) on the 17th, 19th or 21st day (of the Islamic, lunar month) then it is a cure for every disease." [Saheeh Sunan abi Dawud (3861)].

The Best Days For Cupping (Hijama)

Ibn Umar (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "Cupping (hijama) on an empty stomach* is best. It increases the intellect and improves the memory. It improves the memory of the one memorising. So whoever is going to be cupped then on a Thursday in the name of Allaah.  Keep away from being cupped on a Friday, Saturday and Sunday. Be cupped on a Monday or Tuesday. Do not be cupped on a Wednesday because it is the day that Ayoub was befallen with the trial. You will not find leprosy except (by being cupped) on Wednesday or Wednesday night." [Saheeh Sunan ibn Maajah (3488)].
* 'alaa ar-reeq in arabic means to fast until after being treated with cupping. Once the treatment of cupping has been completed, one may eat and drink. 
As for the Islamic day and night, the night enters before the day. So at sunset on Tuesday, Wednesday night comes in. Cupping (hijama) is best performed during the daytime between the athaan of fajr and the athaan of maghrib because yawm in arabic means daytime.  
The Sunnah days for cupping (hijama) every month are when the 17th or 19th or 21st of the lunar month coincide with a Monday, Tuesday or Thursday. These are the best and most beneficial days for cupping (hijama). If one is not able to be cupped on 17th, 19th or 21st (coinciding with Monday, Tuesday or Thursday) then any Monday, Tuesday or Thursday of the month.
Cupping (Hijama) For Women
Jaabir ibn Abdullah y reported Umm Salama (may Allah be pleased with her) asked permission from the Messenger r to be treated with cupping (hijama). So the Messenger r ordered Abu Teeba t to cup her. Jaabir ibn Abdullah y said, " I think he r said that Abu Teeba t is her brother through breastfeeding or a young boy who didn't reach puberty". [Saheeh Muslim (5708), abu Dawud (4102), Saheeh ibn Maajah (3480)].

Dry and massage cupping may be performed at any time.

How Can Bleeding Possibly Be Beneficial?

The following is an article from Dr Rizwhan Suleman (Mchiro) of Hijama Centres UK.

What is Cupping/Hijama?
Cupping is the modern term for an ancient medical treatment in which a partial vacuum is created in cups that are positioned onto the surface of the body. The vacuum created by either heat or suction draws up skin, subcutaneous tissue and muscle layers a few millimeters into the cups.


Hijama or wet cupping is also an ancient medical technique popular amongst Arab cultures. It utilizes the same principles of regular cupping therapy except it allows the release and removal of stagnant blood, toxins, inflammation and swellings through the application of small scratches or pin pricks over the areas where the suction is applied.


This combination of cupping therapy and controlled bloodletting has been utilized for over 3000 years and was a practice the prophet Muhammad (pbuh) promoted & recommended as an inspiration from God. The practice of wet cupping was also an integral part of more recent medical practice as it developed in the Middle East and was used up until the 1800’s even here in the UK.


Although a simplified explanation wet cupping can be likened to a mechanical form of leaching allowing small quantities of blood to be released from swollen and painful areas of the body, to give relief, promote healing and to regain function.


How Can Bleeding Possibly Be Beneficial? 
Many conditions that we suffer with are related to inflammation. This includes all conditions that end with the word “itis” such as arthritis, tendonitis, gastritis, appendicitis etc. The common factor in all these conditions is that there is too much unwanted inflammation in that area. This is why anti-inflammatory medication is regularly prescribed to try and reduce down the amount of this inflammation.


Unwanted inflammation can be simplified as a state in which there is too much blood in an area causing pressure and stagnation preventing the regular flow of nutrient and waste exchange, resulting in pain and compromised natural healing abilities. If left untreated this can develop into a chronic state in which the reduced blood flow allows the build up of waste, carbon dioxide and acidity, causing further pressure, cell damage, pain, and discomfort leading to more inflammation, perpetuating the problem.

Why use Hijama? 
Chemical drugs can be used to reduce pain and dampen inflammation but cupping therapy has the unique ability of drawing out inflammation, stagnant blood, cell debris and toxins from deep within the body to the surface of the skin.


By utilizing wet cupping this built up pressure can be released through tiny pin pricks or light scratches on the skin which often gives instantaneous relief and allows fresh blood, nutrients and oxygen to penetrate the area breaking the cycle of pain and discomfort.


Traditionally it has been understood that wet cupping also allows a process of purifying the blood and removing unwanted materials from the system. By removing these elements you can reduce the problems that they cause and reduce pain, discomfort and inevitably disease. For this reason wet cupping therapy has been used throughout history to treat a wide range of conditions and even as a preventative medicine.



What can Cupping/Hijama help?
The Prophet (saw) used hijama as a method of detoxification; after ingesting poison, treating hip pain, as a treatment against black magic, for his ankle after twisting it and he would also recommend it to people with Migraine. He (saw) would also use it regularly without having any problems, as a preventative medicine. Throughout history hijama has been used to purify the blood & for a wide range of conditions ranging from infertility & skin problems to gout & arthritis.
 
Is it Safe?
 When practiced in a clean & hygienic environment under the guidance of a qualified Health Professional hijama is very safe & relatively risk free.


Unlike other forms of medicinal interventions hijama does not bring anything foreign into the body eliminating the chances of having a “reaction” to the treatment. It simply acts by restoring your body’s ability to repair and function to its optimal level.


Practitioners can screen for various problems & complications before performing the treatment. This involves checking blood pressure, medication & previous medical history, allowing referal of complicated cases to a senior physician when indicated. Although the quantity of blood removed in hijama is relatively minimal, iron levels and anaemia are always taken into consideration. Materials used to perform the treatment are all single use & disposed of after each patient.


When can hijama be performed?
It is the consensus of the scholars that like any medicinal treatment hijama should be used as & when needed. This is how the Prophet (saw) used it when treating health problems. There are ahadith that mention a preferred time of the month (17th, 19th & 21st) however the prophet (saw) also performed it out of this time such as while on Hajj. There are no dates, times or days when hijama is not permitted.

Hijamah in Jeddah, Saudi Arabia - by Zeshan Ali Khan

This is a guest-post in the form of a written interview with Zeshan Ali Khan - a male cupping therapist based in Jeddah, Saudi Arabia.


Please give us a short introduction about yourself, and in particular how you ended up being hijama therapists?

My name is Zeshan Ali Khan, I am from Pakistan. I did Masters in computer sciences from Gomal University D.I.Khan Pakistan in 1998. After completing my education I got work visa for Saudi Arabia in August 2001 and finally settled in Jeddah.
Now recently I am working in IT department of one of the retail business company in KSA.
We have a history of lower back pain and head-ache as a family problem, and I recently suffered from these conditions as well. For that, I had so many treatments; also I had my MRI which was very normal. One of my friends introduced me to Hijamah. Still I was reluctant to have Hijamah therapy for myself. Instead I got proper training from Brother Abu Abdul Wali, as my wife was more interested than me to learn Hijamah and practice it, so that is why she also got proper training from Umm Abdul Wali.
And I am sure that Brother Abu Abdul Wali and his wife both are one of the best Hijamah therapists in this world. He is not only my teacher but a good friend also. I saw and did so many cases with my teacher. And almost all the patients obtained cured with great results.
So after learning, my wife did my Hijamah for the first time, she applied three cups 2 on Akhda'ain and one on Kahil. And from the very first session I was feeling relief from backache. Alhamdolillah
And why not because this is a Sunnah of our beloved Prophet Mohammed (Sallallaahu Álayhi Wasallam), which was the main point that forced us to learn Hijamah.
- What particular illnesses/ conditions (if any), do you specialize / had the most experience in?
For the lower back pain and usually it is related to spinal Cord, I cupped lot of patients complaining lower back pain. And 8 cups plan on back gives a sound result in this scenario.
- And do you find that in your experience, the conditions/illnesses that you have come across respond well to cupping (hijama) therapy? And if so, what are they?
In all my case list for all patients except one OR maybe two had positive results starting from the next day of Hijamah. And as per my experience for migraine, leg pain and backache Hijamah is like a pouring of water on fire, and of course healing always comes from the sky.
- Do you have any interesting case-studies that you can briefly share with us from your hijama treatments?
I have so many cases in my case list. The most common problem for which patients come to me is Backache, for which I apply 2 cups on Akhda'ain, one on Kahil and 2 cups along the backbone and 2 cups bilaterally in the lumber region. This plan of cupping usually helps in this problem.
And the second case of migraine responds well to 2 cups on the scalp Yafookh and Haamah. These 2 points are the best location for migraine and I saw many patients recovering from migraine after applying 2 cups on the scalp.
And the 3rd case which I experienced with very good results was a case of Allergic Asthma. In these patients I use to apply 5 cups. Two on Akhda'ain one on Kahil and two bilaterally on the back in the lungs region.
And also for the patients who use to complaint for the knee pains, I apply 3 cups on each knee, that is left, right and on the top of knee. One of my friend was complaining for knees pain especially during Salaat in Rukoo'. After applying 6 cups on his both knees, it gave tremendous result and patient recovered after first session Alhamdolillah.
But for sure the healing always comes from the sky.
- Also what would you say about using cupping as a general preventative therapy and how often would you recommend this (once a month? once/twice a year?)
As Hijamah is the beautiful Sunnah of our beloved Prophet Mohammed (Sallallaahu Álayhi Wasallam) so for us being a Muslims there should not be any doubt to adopt Hijamah therapy as a first aid. And as per my experience one should repeat Hijamah after six months especially for the spinal related issues.
- As a hijama therapist, how often do you get hijama done yourself, and what for (if any specific condition) and who do you go to get it done (since it is not an easy therapy to self-administer)?
Of course 17th, 19th and 21st of the Islamic months are the best days for Hijamah, but one can perform it on any other days based on requirements.
For my lower back pain issue once in six months I use to have Hijamah performed by my wife. My wife has become an expert in Hijamah therapy, as she has done lots of cases. By profession, she is a medical doctor but recently, she is practicing Hijamah exclusively.

- What is the overall prevalence of hijama therapy in your country at present? 

 
In Saudi Arabia I think that local Arabs are already familiar with Hijamah, & expatriates are also having the awareness of this forgotten Sunnah. And I believe that my teacher brother Abu Abdul Wali has played crucial role and of course he is one of the pioneer for spreading this forgotten Sunnah among Indo/Pak community in Saudi Arabia. May Allah reword him the best as he deserves.
Is it now available in hospitals and clinics as a valid alternative / sunnah therapy?
 

As currently I am staying in Saudi Arabia, one of my friends told me that there are some hospitals probably in Makkah region practicing Hijamah therapy. But personally I didn’t see any Hijamah clinic around.
But in Pakistan there are many clinics that are using Hijamah as a valid alternative/Sunnah therapy.

- Is there anything else you would like to add?
As per our beloved Prophet Mohamed (Sallallaahu Álayhi Wasallam) said, "Indeed the best of remedies you have is Hijamah".
So I request to all readers that they must learn Hijamah. Specially teach Hijamah to your sisters, mother and wife, as we are short of female Hijamah therapists. So it is a good practice that a man should cup the male and a woman should cup the females.

The Sunnah day for wet cupping (hijama) in this month of Dhul Qida is as follows:
  
Monday 19th Dhul Qida 1432 
Monday 17th October 2011


Please kindly note that these dates are based on the new, lunar month according to the sighting of the new moon by the naked eye, as cupping (hijama) is a form of worship. These dates may sometimes vary from the calculated, lunar dates.

Cupping (Hijama) In Its Time Is A Cure For Every Disease


Abu Hurairah (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "Whoever performs cupping (hijama) on the 17th, 19th or 21st day (of the Islamic, lunar month) then it is a cure for every disease." [Saheeh Sunan abi Dawud (3861)].

The Best Days For Cupping (Hijama)

Ibn Umar (may Allaah be pleased with him) reported that the Messenger (Sallallaahu Álayhi Wasallam) said, "Cupping (hijama) on an empty stomach* is best. It increases the intellect and improves the memory. It improves the memory of the one memorising. So whoever is going to be cupped then on a Thursday in the name of Allaah.  Keep away from being cupped on a Friday, Saturday and Sunday. Be cupped on a Monday or Tuesday. Do not be cupped on a Wednesday because it is the day that Ayoub was befallen with the trial. You will not find leprosy except (by being cupped) on Wednesday or Wednesday night." [Saheeh Sunan ibn Maajah (3488)].
* 'alaa ar-reeq in arabic means to fast until after being treated with cupping. Once the treatment of cupping has been completed, one may eat and drink. 
As for the Islamic day and night, the night enters before the day. So at sunset on Tuesday, Wednesday night comes in. Cupping (hijama) is best performed during the daytime between the athaan of fajr and the athaan of maghrib because yawm in arabic means daytime.  
The Sunnah days for cupping (hijama) every month are when the 17th or 19th or 21st of the lunar month coincide with a Monday, Tuesday or Thursday. These are the best and most beneficial days for cupping (hijama). If one is not able to be cupped on 17th, 19th or 21st (coinciding with Monday, Tuesday or Thursday) then any Monday, Tuesday or Thursday of the month.
Cupping (Hijama) For Women
Jaabir ibn Abdullah y reported Umm Salama (may Allah be pleased with her) asked permission from the Messenger r to be treated with cupping (hijama). So the Messenger r ordered Abu Teeba t to cup her. Jaabir ibn Abdullah y said, " I think he r said that Abu Teeba t is her brother through breastfeeding or a young boy who didn't reach puberty". [Saheeh Muslim (5708), abu Dawud (4102), Saheeh ibn Maajah (3480)].

Dry and massage cupping may be performed at any time.
